NECPA IPEC Collaborative Care Summit

2019 Facilitator Page

Thank you again for agreeing to be a facilitator at the Collaborative Care Summit. This page will provide all the information you will need to make this a successful event. As noted in our email communication to you, a brief orientation will occur before the start of the summit. You will be directed where to go for the orientation when you arrive. 

The student learning objectives for the summit are to:

  1. Demonstrate interprofessional collaboration in the direct care of a patient. (RR5. Use the full scope of knowledge, skills, and abilities of professionals from health and other fields to provide care that is safe, timely, efficient, effective, and equitable)

  2. Communicate with team members regarding each person’s role for the patient in the context of their training and profession. (CC4. Listen actively, and encourage ideas and opinions of other team members)

  3. Readily change roles and assume responsibilities best suited for a situation based on professional scope of practice (TT11. Perform effectively on teams and in different team roles in a variety of settings)​​​

This is THE critical document!  This document contains the case and facilitator guide. Click on the image to the left and download this case. You should review prior to the Summit. Bring a copy with you to the Summit.​

Keep in mind, each small group will contain a different "assortment" of professional students. As a facilitator, you need to be prepared to fill in sometimes. 

Training Module

 

This training video will run you through an abbreviated version of the simulation and show you how to use the facilitator guide to successfully discuss the case. Read through the guide first, then follow along as you watch the video. Click the image on the right to view the training video.

The video is about 35 minutes in length . Please watch this video and sign the following attestation of completion before the summit.

In case technology does not cooperate and you cannot view the videos above on the day of the simulation, the scripts for each video are available from the link to the left. Bring a copy with you to the Summit! Click the image to the left.

Naloxone Information Sheet

 

As the simulation is based on a case of opioid abuse, some questions will come up regarding the use of naloxone. The link to the right contains an information sheet from the Pennsylvania Department of Drug and Alcohol Programs.​ This information will be made available to students AFTER the summit. An Overview of the Health Professions

 

This guide was created by the NECPA IPEC. It describes the various professionals involved in the summit. This will provide the facilitator a greater ability to understand the various students so that they can be more fully involved in the discussion. Click the image to the left.
